Blepharoplasty is the procedure through which we improve the appearance of the periocular area. It can be performed on both the upper and lower eyelids. Thanks to this procedure, we can reduce dark circles and the appearance of tired eyes.
Upper blepharoplasty is a procedure that allows us to modify excess skin in the upper eyelids, reducing the characteristic laxity in this area that occurs over time. We can also excise the excess fat in the bags that appear in this area, creating a puffy appearance in the eyelids. It is important to highlight that, like all procedures in plastic surgery, there are clear and precise indications. The scar is virtually imperceptible once the healing process is complete, as it is located along the crease of the eyelid.
Lower blepharoplasty allows us to address various conditions that may or may not be present in some patients. It is possible to modify excess skin in this area, which usually creates very fine and bothersome expression lines for patients. Fat bags that create a tired appearance can also be removed. Not all patients require the excision of skin or fat bags; each case is individual and will be determined during the initial assessment. The approaches can be made under the eyelashes, being completely imperceptible, or transconjunctival, on the inner part of the eyelid. The approach to be used will be determined based on each patient’s case.
The healing process in this anatomical area is very favorable, and pain is almost negligible. However, it is normal to experience some swelling or edema around the eyes, some bruising, and slight dryness. The required recovery time is approximately five days, after which patients can return to normal activities with certain restrictions: heavy lifting and prolonged bending should be avoided. The use of sunglasses, hats, or other protective items will prevent sun exposure from causing stains on the skin while there is still some bruising.

When is it indicated?
This procedure is indicated for patients who:
- Desire to improve the aesthetic appearance of the periocular area
- Have tired-looking eyes
- Experience excess fat in the eyelids
- Have excess skin in the upper and/or lower eyelids
- Have asymmetry in the eyelids
- Have many wrinkles or expression lines in the lower eyelid
- Have visual field obstruction due to excess skin in the upper eyelids
How is it performed?
It is important to always conduct a pre-surgical consultation to determine the patient’s requirements, expectations, and conditions before the procedure. After this, the surgical intervention is performed in an operating room with complete asepsis and antisepsis.
UPPER BLEPHAROPLASTY
We make precise markings of the tissue to be removed during the procedure, taking exact measurements. Subsequently, we incise along this marking, remove the excess skin previously outlined, excise a strip of muscle, and dissect the planes until we reach the fat bags. Some of these bags are removed while always leaving a portion of them. We then verify that there is no residual bleeding and close the tissue in layers.
LOWER BLEPHAROPLASTY
An incision is made below the lower eyelashes or in the conjunctiva. Skin resection is performed if necessary, and the planes are dissected until reaching the fat bags, which are either excised or repositioned depending on each patient’s case. In some cases, a canthopexy (repositioning the outermost part of the eye) may be required, securing the lower eyelid tissue to the lateral border of the orbit by designing and dissecting special flaps. The anatomical planes are closed, and the procedure is completed.
PRE-SURGERY INSTRUCTIONS
- Do not take anti-inflammatory medications such as aspirin, ibuprofen, or diclofenac, as these can affect blood clotting and may increase bleeding during your surgery. If you experience any pain, you may only take acetaminophen.
- Please notify us if you are taking any herbal medications, vitamins, or natural supplements, as some may cause complications during your surgery. Ideally, discontinue them at least one week before the procedure (e.g., Ginkgo biloba, Omega-3, multivitamins).
- If you have a common cold, injury, or any other condition during this period, please notify us.
- Smoking can affect your healing process, please stop smoking at least 15 days before the procedure.
- One week before surgery, you should attend a pre-anesthesia assessment so that the anesthesiologist can clarify any questions regarding the type of anesthesia to be used and sign the necessary consent forms.
- You should meet with us to clarify any questions regarding the procedure and define the size of any implants.
POST-SURGERY INSTRUCTIONS
- Apply cold compresses to the eyes for the first two days to reduce inflammation and potential bruising.
- Keep your head elevated for a week—semi-sitting position.
- Wash and care for the affected areas, strictly following the doctor’s instructions.
- Apply Tobradex three times a day inside and outside the treated eyes.
- Avoid watching television or using electronic devices (phones or computers) for the first 24 hours post-surgery.
- Refrain from applying makeup for the week following surgery.
- Avoid sun exposure for at least one month after the procedure.
- Take prescribed medications (mandatory antibiotic intake!).
- Avoid strenuous activities such as mopping, sweeping, ironing, lifting children, or heavy objects for one week.
- Remember to attend the scheduled follow-up appointment at the clinic.
